I can upload (file appears on server) but somehow Pivot doesn't generate a <img> code at all in the entry. Once I select a file from the list and use it that way, all of a sudden it does show that image. What can this be?

No idea, but maybe you should first upgrade to 2.2.5. There were a lot of issues with the uploader in previous versions.

I am at 2.2.5 now, but still the same problem. It's not even a problem really but more an inconvenience, instead of clicking through the regular process, after uploading you have to re-find the file in the list... :S

Still some additional information would be useful. From what part of editing an entry are you uploading an image? Extra image? Gallery? Does this also happen in another browser?

Entry, but similiar problems with Bonus fields. It also happens in another browser.

I figured out that newly uploaded images have a chmod of 0644 or something similar and when I change that manually to 777 it works. I looked through Advanced Configuration and there is a chmod entry there for 0644 but another Pivot installation has that as well and works perfectly. Sooo, where does it decide to chmod new images wrong?

You did chmod folder images (and everything beneath it) to 777 after installing? Maybe there is something wrong with the ownership of the folders?

Yes of course, but newly uploaded get 0660. First I said 0664 but that would've been perfect, because than they'll work. But they get 0660 and that's why they don't work.

Probably the shitty host? Safe mode maybe? Can't find any setting in Pivot which would do this.

Please check Dashboard: Administration / Configuration / Debug / Chmod files to. It should read 0644.

I'm not sure what you mean. Can you make some screenshots to illustrate what's going wrong, exactly? (and information about browsers use would be helpful)
